However an inlying catheter for a period of two or three weeks following castration frequently relieves the patient of his urinary retention and so obviates the necessity of a suprapubic tube. The modern trend therefore is to avoid any surgical treatment of the prostate itself (in inoperable cases) until adequate hormonal control has been tried.

When no further benefit can be obtained by hormonal control, when the patient is becoming bedridden and has so much pain that he gradually is becoming addicted to narcotics, when the disease is gradually progressing, especially in bone, I have found that the use of intra venous injections of radioactive phosphorus almost always relieves the patient of his pain, and may rehabilitate him for a period of three or four months, or even up to a year or more after each course of treatment.

To treat advanced inoperable carcinoma of the prostate I usually stop the estrogens and give the patient 100 mg of aqueous testosterone intramuscularly daily for three days, and then give him 1 to 1% millicuries of radioactive phosphorus intravenously daily until he has had from 10 to 15 millicuries.

How much he gets depends on his relief of symptoms and his response to the drug. The 100 mg of aqueous testosterone is continued daily throughout the treatment and for five days following the treatment. At the end of this time he resumes his estrogen therapy

More extensive involvement of the bladder with one or multiple tumors may require a total cystectomy, but when this is done it must be borne in mind that one assumes that the growth has already extended beyond the bladder, and any expectation of cure is therefore eliminated. Consequently the operation for total cystectomy is primarily a palliative one, and should be done for the tumors of multicentric origin which have not yet become infiltrative. This type of tumor also sometimes responds to intracavity irradiation with a capsule of radium or of radioactive cobalt, or the use of a radioactive Bromine solution within a bag.

This type of intracavity irradiation however has been relatively unsatisfactory and more recently better results, or equally good results, have been obtained by the use of he cobalt bomb or, as it is more correctly known, tele cobalt radiation therapy. I personally prefer the use of interstitial radioactive Cobalt (Co ) which gives all the benefit of 60 total cystectomy without the necessity of urinary diversion.
