Pleural mesothelioma is an aggressive form of cancer that attacks the mesothelial cells of the pleura, a membrane that covers the lungs and lines the chest cavity. Pleural mesothelioma accounts for approximately 75% of all mesothelioma cases. Pleural mesothelioma is most commonly unilateral (it affects only one side of the chest cavity}and it occurs most commonly in men aged between 60-80 years of age with a history of previous asbestos exposure, often decades prior to diagnosis.

When pleural mesothelioma begins, it appears as a series of small white nodules which become diffuse, or widespread on the pleural surface. Gradually, these nodules begin to grow together and thicken, forming a rind that encases the lung and extends into the fissures or grooves of the pleura and the diaphragm. The tumor spreads by direct invasion of surrounding tissue, inwardly compressing the lung, and outwardly invading the chest wall and ribs.

Symptoms

The early symptoms of pleural mesothelioma are not, as a rule, specific enough to cause alarm, and in most cases are attributed to increasing age or overwork. Because of this, it may be several months from the onset of generalized symptoms until the first acute symptom, pleural effusion, occurs. At that time, progressive shortness of breath caused by the effusion, and chest pain caused by chest wall invasion may be in evidence. More general symptoms include dry cough, fatigue, night sweats and weight loss.

Diagnosis

On initial examination by a doctor, 80% to 95% of patients show pleural effusion on x-ray, the remaining percentage show little or no fluid. At first, the fluid is free-flowing, and is similar in appearance to that seen in other benign causes or in congestive heart failure, and because of this, these other possibilities are the first to be ruled out in the diagnostic process. Later, the effusion becomes loculated, or contained within a boundary in the pleural space, where the fluid does not move.

CT scans are more definitive, and may show not only the effusion, but the presence of pleural masses as well as the size certain lymph nodes; MRI is more sensitive in determining chest wall invasion and spread of disease through the diaphragm; PET may help in staging the disease for possible surgical resection by ruling out extension to the contralateral (opposite) lung or to other distant sites.

Analysis of pleural fluid yields a confirmed diagnosis in a relatively small percentage of patients, and needle biopsy offers only slightly better results. Today, the procedure of choice is the VATS (video-assisted thoracoscopy) procedure, which has a diagnostic yield of >95%, and allows for pleural biopsy, drainage of fluid and pleurodesis. VATS also ensures adequate tissue samples to facilitate a definitive diagnosis.

Staging

As with any type of cancer, staging plays a role in what treatment options a patient might be eligible for. Several staging systems are in use, however, the most widely used and most comprehensive is the TNM system associated with the International Mesothelioma Interest Group.

Treatment

Based on different of factors, several treatment options may be available including surgery, chemotherapy, radiotherapy (or combinations of all three, known as trimodal therapy), clinical trials, gene therapy immunotherapy etc. Surgery is used to remove as much tumor as possible and chemotherapy and or radiotherapy is used to eliminate the remnant cancer cells. Once a diagnosis is reached, it is important that all possible options are discussed by the appropriate doctors with the patient and their family. Although pleural mesothelioma continues to be a difficult cancer to treat, more awareness of the disease, new and better diagnostics, and more successful treatment regimens help to improve its outlook.

There have been some few patients who have survived far beyond the usual one year prognosis for most mesothelioma victims and a handful that have even been cured, with no trace of the aggressive cancer several years after treatment (though recurrence is always possible).

There seems to be one common factor amongst those that have survived the disease for longer times -a boosted immune system. Studies of those who have either survived or been cured of the disease reveal that most of these patients participated in some sort of therapy that enhanced their immune system. Some treatments included clinical trials in immunology while others involved alternative therapies dealing with the immune system.

A number of mesothelioma survivors have posted their stories on the Internet and have spoken to patients and their families with the hope of providing a brighter outlook for their future. These same survivors have also presented their cases to doctors and researchers. The most outspoken of these individuals is Paul Kraus, an Australian mesothelioma survivor. Kraus was diagnosed with abdominal/ peritoneal mesothelioma in 1997, a form of the disease that is tougher to treat than the more common pleural mesothelioma. Kraus worked in a factory 35 years earlier where he was exposed him to blue (crocidolite) asbestos.

Upon his diagnosis, Kraus changed his diet (he’s now a vegetarian), added many vitamins and supplements to his daily routine, began “juicing,” and tried a treatment known as ozone therapy. Kraus explains, “They took blood out of a vein, used an ozone machine to add ozone (a molecule composed of three oxygen atoms) to the blood and reintroduced it into my body through a drip. The rationale for that was that cancer does not like an oxygenated environment.” The research in putting the protocol together took a lot of time, but in the long-run, Kraus believes the combination of all these treatments stabilized his mesothelioma. Today, tests show he still has the disease and his body is a bit weaker than it was, but he has no pain and his doctors tell him he has many more years to live. Kraus stresses the fact that he is not the only mesothelioma survivor.

He has cited the case of a man diagnosed 14 years ago at age 58 (no name given) who had a chest wall resection after diagnosis and has had no symptoms or recurrence since. His doctors believe there was that spontaneous regression he experienced may be an immune-mediated phenomenon, that is his immune system played a role in his survival.

Another survivor, Rhio O’Connor – who recently published a book about his fight with mesothelioma – has lived with the disease for seven years. He opted against the conventional methods of treatment like surgery, chemotherapy and radiotherapy. Instead, with the help of several medical physicians, he developed a regimen that included 100 supplements per day, changed his diet drastically, and now practices what he calls “mind-body medicine” (such as non-stress techniques like meditation). In his book, he recommends a variety of alternatives to traditional medicines and treatments.

All mesothelioma patients should be encouraged to participate in clinical trials especially those that are trying out new methods of boosting the immune system.

Exposure to asbestos fibers has been strongly linked with the development of mesothelioma. At some point in our lives, most of us have been exposed to asbestos in the air we breathe and the water we drink; from natural deposits in the earth, and from the deterioration of asbestos products around us. Asbestos was used commercially in North America as early as the late 1800s, but its use increased dramatically during the World War II era when shipyards produced massive numbers of ships for the war effort. Since that time, asbestos-containing products were used by the construction and building trades, the automotive industry and the manufacturing industry. All told, more than 5,000 products contained asbestos.

For more than 50 years, products containing asbestos remained unregulated, and the manufacturers of those products continued to prosper, knowing full well that many of the millions of workers who came into contact with their products would ultimately suffer as the result of their actions. Finally, in the late 1970s, the Consumer Products Safety Commission banned the use of asbestos in wallboard patching compounds and artificial ash for gas fireplaces because the fiber could easily be released during use. In 1989, the Environmental Protection Agency banned all new use of asbestos, but uses established prior to that time were still allowed. Although increase in public awareness about the dangers of asbestos and public concern over the issue has led to a decline in domestic consumption over the years, a total ban on asbestos has not yet been implemented. Asbestos is still imported, still used and still very dangerous.

What is Malignant Mesothelioma?
